Certificate of Insurance (COI)
You must submit your COI and be sure to meet ALL of the requirements from each of the following categories: verbiage, certificate holder, general liability limits & auto liability limits. If you do NOT meet all of the requirements listed here, your application will be on hold and considered INCOMPLETE until updated .
In the “DESCRIPTION OF OPERATIONS/LOCATIONS/VEHICLES/SPECIAL ITEMS” section:
“The State of Hawaii, including all of its departments and attached agencies, their officers, employees and agents are named as additional insured with respect to the named insured’s filming activities.”
In the “CERTIFICATE HOLDER” section:

- The “insured” on the Certificate of Insurance must be the same as the “Applicant” on the film permit applications.
- The University of Hawaii must also be named as additionally insured when filming at any of its facilities.
- Kaka’ako Waterfront Park is no longer an Open and Accessible Site on Oahu. The Hawaii Community Development Authority (HCDA) must also be named as additionally insured when filming at Kaka’ako Waterfront Park.
Minimum Requirements for a STANDARD FILM PERMIT
- General Liability ($1 million per occurrence, $2 million aggregate)
- Automobile Liability: *preferred ($1 million bodily injury per person, $1 million bodily injury per accident and $1 million property damage per accident)
- Naming the State of Hawaii as additional insured, is required for a Standard Film Permit.
NOTE:If your production cannot provide the State’s preferred auto liability limits, either of the
following 2 options can be accepted. These alternative options refer to Auto Liability only NOT General Liability.
Option#1:If the production has combined single limit of $1 million, they can provide excess (or umbrella) liability. They only need to provide $1 million of excess liability.
Option#2: If the production has combined single limit of $1 million, they can increase the combined single limit to $2 million to meet the State’s needs.

*Per DLNR, automobile liability is no longer required for an Open and Accessible Permit.

Minimum Requirements for Drone Use
Evidence of coverage for the use of Unmanned Aircraft Systems (UAS) provided by the UASoperators. Each UAS operating entity must submit proof of Unmanned Aircraft Systems (UAS) insurance for owned and/or non‐owned aircraft with a limit of at least $2,000,000 per occurrence which names the State of Hawaii as an additional insured.
This can be submitted in one of two ways:
- If UAS coverage is being added to a general liability policy, there must be a separate endorsement showing proof of UAS coverage or the UAS coverage needs to be evidenced on the certificate.
- If the coverage is coming from an aviation‐specific insurance carrier (such as Global Aerospace), a separate policy endorsement is not necessary, as the entire policy represents the UAS coverage
If a separate endorsement is needed, it must name the State of Hawaii as Additional Insured on the policy using our required wording.
